Medically Tailored Meals

The Regional Food Bank of Oklahoma (RFBO) leads the Medically Tailored Meals program, providing dietitian-designed, scratch-made frozen meals at no cost to eligible adults living with heart disease and/or diabetes who also experience food insecurity. Participants receive ongoing meal support for up to nine months, with meals distributed through their healthcare provider for convenient home storage.

OFMQ partners with RFBO by delivering disease management education and tailored health education classes to support improved health outcomes and reduce hospital readmissions. Together, this initiative addresses both nutrition access and chronic disease management to support long-term health and wellness.

Education & Program Delivery

Participants receive evidence-based diabetes and chronic disease education using interactive tools such as Conversation Maps, ADA- and DSMES-certified classes, self-monitoring blood pressure lessons, and practical care kits. Each session includes supplemental materials—such as printed resources, MyPlate guides, and take-home tools—to support learning and behavior change.

Education is offered through flexible virtual and in-person formats, with scheduling and session structure tailored to meet the needs of participating facilities and individuals. Instruction incorporates motivational interviewing, self-help coaching, and supportive encouragement to promote sustainable mental, emotional, and behavioral health improvements.
