Age-Friendly Health Systems

This project supports hospitals in adopting Age-Friendly Health Systems best practices by facilitating implementation of the 4Ms framework—What Matters, Mobility, Mentation (Mind and Mood), and Medications. Through in-person and virtual training, data analysis, workflow optimization, EHR enhancements, and targeted educational resources, the initiative helps organizations improve quality measures, reduce high-risk medications, enhance safe mobility, and strengthen cognitive screening and interventions. The project also guides hospitals toward Institute for Healthcare Improvement (IHI) Age-Friendly Health Systems recognition, supporting achievement of up to two recognition levels based on readiness and data maturity.



OFMQ supports the Oklahoma State Department of Health in advancing Age-Friendly Communities by partnering with rural hospitals to build a sustainable Age-Friendly ecosystem. Our team facilitates implementation of the Age-Friendly 4Ms framework and provides hands-on technical assistance to help rural hospitals adopt best practices, strengthen care delivery, and achieve Age-Friendly Health Systems goals.
