Quality Improvement

Receive assistance with health information technology that is focused on maximizing data quality improvement and excelling in areas of need via:

  • Patient portals and HIEs can ease the burden of paper processes typically utilized in forms and referrals between care settings.  OFMQ will assist each practice with the engagement of patients using promotion of the portal to enter and view their health data.   

  • OFMQ aims to help increase the use of technology for patient use by enabling patients to make smart and educated decisions regarding their healthcare. Patients feel more empowered when they are provided with the knowledge, skills, and ability to manage their own care.   OFMQ will create, discuss, and aid in implementing interventions designed to increase the use of patient engagement tools and support positive patient interactions. 

  • Providers are required now to submit data for quality reporting programs like the Uniform Data System (UDS) or the Merit-based Incentive Payment System (MIPS).  OFMQ will help verify that you have an accurate reflection of your data within the HIE, or third-party reporting software applications used by your practice and make recommendations, such as chart abstraction, for testing validity of the data. 

  • Multiple third-party reporting software systems or even an HIE can be utilized to help with data capture and retrieval. The key to success is utilizing the functionality within these systems to improve patient care and data. If you cannot find a report, then you may be able to pull the information needed with registry functionalities from your EHR. Ask yourself: what reports are you using on a daily, weekly, or even monthly basis to ensure better quality for your patients, seek out weaknesses to improve upon, or simply to just ensure efficiency in your clinic. OFMQ will produce a plan to better utilize the data your providers and staff are collecting on a regular basis.
