Telehealth

Telehealth is the remote care delivery or monitoring between a healthcare provider and a patient outside of a clinical facility, such as in the patient's place of residence, either through phone monitoring (via the telephone) or telemonitoring (through electroinic information processing technologies). 

Telehealth is a practice tool that can:

  • reduce home care visits
  • reduce avoidable hospitalization
  • increase self-management by patients
  • increase access to specialized health care to rural areas
  • allow real time medical consultation to patients living in rural locations
  • increase efficiencies and reduces costs for providers and for patients
