The Quality Improvement Organization (QIO) Program, under the oversight of the Centers for Medicare & Medicaid Services (CMS), has developed the Setting Targets – Achieving Results (STAR) Web site, an interactive site for nursing homes and home health agencies to set and track annual targets for publicly reported outcome quality measures.

Any agency or nursing facility can register and use the site at no cost. OFMQ assists providers use the site to set targets, track performance over time, identify opportunities for improvement and develop strategies to achieve higher performance.
