Community Care Transitions

Together, we can prevent 5,276 hospital readmissions and save $46m in healthcare spending in our state. Join OFMQ as we help communities measure the impact of hospital readmissions and build a plan for transformational change.

Frequent hospital readmissions are not just a hospital problem, they are a community problem. All providers and caregivers in a community have a role in achieving the high quality, coordinated and patient-centered care that prevents avoidable readmissions. Funded by the Centers for Medicare & Medicaid Services (CMS), this "all participate, all learn" Care Transitions Learning and Action Network (LAN) seeks to connect communities to help achieve the bold goal of reducing avoidable hospital readmissions by 20% by 2014.
