Approximately 419,600 Oklahoma adults (age 18 years and older) have diabetes, or 15.6% of the adult population, according to 2006 data available from the state health deparment. Nearly 20% of Oklahomans age 65 years and older reported a diabetes diagnosis. Diabetes is responsible for thousands of hospitalizations every year.
OFMQ works with physicians and clinics to improve the quality of care for Oklahomans with diabetes. Our team of nurses, doctors and certified diabetes educators provide support and training to physicians and office staff on the latest diabetes management techniques using evidence-based medicine. We offer a variety of provider-directed and self-management tools for patients.
We offer specialized support to physicians using electronic medical records systems to better manage care for diabetes. The quality measures for diabetes care we seek to improve are:
- A1C tested annually
- Eye examination performed biennially
- Lipid profile performed biennially
- Urine protein test annually
