Community-Based Quality Improvement Specialist

This position is focused in the Centers for Medicare and Medicaid Services (CMS) Care Transitions and Patient Safety Theme portions of the contracted scope of work as well as other community-based contracts requirements. This position is responsible for consulting with providers related to community-based projects, improvement of health associated infection projects and implementation of effective quality improvement interventions to improve outcomes associated with these projects.

Responsibilities:

• Recruit providers and communities and provide technical assistance to promote improvement in contract-associated Performance Measures and QI initiatives, particularly those related to improving healthcare associated infection rates
• Work with hospital leadership and community stakeholders in group settings. Facilitate stakeholder meetings and work to maintain net forward energy and obtain concensus
• Establish and maintain effective relationships with multiple provider types, their improvement teams, staff and related groups in order to maintain ongoing engagement in quality improvement projects, provide interventionional solutions and promote cooperative relationships
• Utilize collaborative group and individual consultation models to promote the Quality Improvement process to providers. Assist in tracking progress toward goals, the performance of self measurement, the development of system changes and data interpretation
• Be an integral part of a team that creates and facilitates Learning and Action Network meetings and conferences
• Utilize evidence-based theory to design and measure interventions.

Requirements:

• An applicable clinical license and a minimum of a bachelor’s degree in a healthcare related field
• Experience working in health care settings across the continum, in quality improvement methodology and collecting and interpreting quality data, in community-based organizing and in healthcare transitions
• Knowledgeable in areas of cross-continum healthcare system processes, healthcare acquired infections, community organizing, healthcare transitions, group facilitation and related best practice guidelines
• Excellent presentation and group facilitation skills
